For coward you have to keep running away from battles using escape or flee. Avenger comes if that one character's allies are killed ofter in battle. I'm not sure of the exact numbers, but that's the general idea.

A.Well Yuna must battle her way against every Aeon at the Remiem Temple. This means all Aeons even the hidden ones. then Yuna must preform the Farplane Sendoff for Belgemine. Once that is done. Congrats you have a moon sigil!!!
