This wasn't actually a loss but I sure felt like it was, I was playing PES 4 with my friend and I was leading 5 -0 at the halftime, I though it was a sure win but he managed to pull out a draw, the match ended 6 - 6.
Also while playing capture the Flag in Unreal Tournament 2004 I was killed several times because I thought the opponent was my teammate, that's the last time I forget my glasses.

one of the first times I played X-2, because I was used to leaving X half way through a fight and it being alright - needless to say when I came back there was a big fat "game over" across my screen -.-
