As for the clothing, the extended tissue has gaps around it, and those clothes ain't the tightest, he could probably squeeze into them. And the region looks poorer from what I see in the video. Maybe he does never take the stuff off.
Personally, it looks legit to me, and I cannot find anything to conclusively suggest it isn't...
I hope that guy has luck with that treatment.
