You have to go on a small quest to obtain it. You must first obtain the crown from the depths of the Marsh Cave, then you must travel to the Western Keep to defeat Astos. His defeat nets you the Crystal Eye.

There's the Piscodemons if you step before the chest surrounded by pillars in the Marsh Cave. You can encounter the Evil Eye in the cave with the Levistone. You can encounter the Death Eye in the room where you play the lute, the first step in the room, although I'm not sure if that's a boss. There's Zombie Dragons before stepping on the throne after getting the Rat Tail. There's so many more...
