I'm still stuck on Chrono Chross, this one boss just kills me everytime. I gave up on it, haven't really attempted to finish it in a long while. I was enjoying it until I got to that one enemy though.
I suppose this could be broken up into two categories. Those games that came out before you could save. Im thinking the old side scrollers. Then the newer games where you could save your progress.
For the old side scrollers it would have to be the First Ninja Turtles game for nes. You had 4 lives for the entire game, one for each turtle. I use to get to the rooftop board and I swear to god they put a jump in that game that is impossible!! I have yet to beat the game to this day.
As for the newer games I got stuck on the Cerberus in Devil May Cry 3 for a long time. I might have to revisit this but that is the hardest boss I can think of in any game I have played in the last 5 years. It took me like a week to beat him.

Hardest game I have ever played in my life was Advance Wars 2. The campaign was hard as hell.
Only game that gave me a problem ever from Genesis to now....Devil May Cry 3, the original version. I swear the developers did not have that game tested, they probably just made it and released it lol. Took me forever to beat it on just Normal, then tried it on Hard....toook forever + 2.
I honestly can't remember any old SEGA game I couldn't beat.

I've said it before, but for me, I thought FF III for the DS was pretty difficult. Mainly when you go to defeat the Cloud of Darkness and you have to "run a gauntlet" of other super powerful enemies before you get to her. Oh, and there's a pretty limited supply of magic restorative items that you are given, so you have to watch that...it doesn't do me any good when I have mage characters who have no MP left. xD
I'll have to agree with DMC III. I bought the Special Edition, which was made easier than the first release, so I thought I stood a chance.... Nope. Managed to get to the part where...

Last one for me would be Sonic 3. It wasn't too hard, until I got to this one carnival stage. There was a certain area that had a spinning cylinder thing and when you got to it, a blockade would leave you trapped on top of it and all I figured out how to do to get out of there was keep jumping up and down to move the cylinder thing. I never figured out how to exactly get further than that, so time would run out while I was making Sonic (or Tails) jump around like an idiot. And yes. I tried to get Tails to fly out of the place too, but he would tire out before long, and I still would be stuck in there.

I think all the Ninja Gaidens were the hardest games that i've played. Can't think of anything else. Well as far as rpgs go, the Ys games were pretty hard too.

This would be my exact answer. It took a lot of time and patience to beat the earlier ones, still, the ones on NES were at least somewhat manageable. I cursed Contra III : Alien Wars, and Japanese for being the only ones who could use the cheats. No matter how mad skills I had I was never able to beat it, definitely the hardest game I played so far.
Out of the new ones I don't remember any game being particularly challenging.

-Sonic the Hedgehog for Xbox. Actually, I didn't want to finish it because I forgot the controls.
-Do PC games count? Anyway, Touhoumon. I picked Chibi Sakuya, and I got whopped by Suika pretty bad. Not until she was level 18, which took nearly forever to reach.

Haha, thats because its so goddamn glitchy... So Sonic The Hedgehog is a game that I found hard, another one would have to be The Legend Of Zelda, Ocarina of Time... not the whole game... JUST THE F***ING WATER TEMPLE! I've had that game for years, put it down multiple times just because of that temple >.<

The hardest game I ever played would have to be Valkyrie Profile 2, this is one game that the early gameplay is hard to defeat and this is a game you don't want to end up with a random battle in the game.

A particularly difficult part on Baten Kaitos for the GCN has me stumped for several weeks. The game in general could be pretty difficult to master. Then again I've never been all that good at card based RPG's. The part I was stuck at involved me taking on Giacomo and his cronies aboard a ship from which there is no escape. This meant that I had to beat them on the poor level I had at the time, with the poor cards I had. It was also fairly near the end of the game (I think) which meant no re-starts. I did do it eventually though. Just had to make sure he couldn't heal or something like that.
Phantasy Star Online was another game for the GCN that could be tricky. The only thing that made the game tolerable in its difficulty was infinite lives. You could die and go right back into combat after stocking up. You basically just took it from where you left off, too. This game was awesome offline with friends. It was also the only game (besides the failed Mario Kart: DD online to my knowledge) that boasted GameCubes online capabilities in England. Online it was really hard, you could go through a boss mode that lasted a few hours and was basically a last stand between characters and bosses. Hard, but awesome.
The secret of monkey island for the PC was a hard, puzzle solving game. That took me months to complete and it was only NES quality graphics.
Oh, also from this era on PC was Eagle Eye Mysteries. On this you went around a town solving minor crimes. This could really bring on frustration through difficulty at times. Especially when it turned out to be the most obvious person and you'd been gathering extra evidence for hours.
Dragon Age: Origin on the PC, too. That damn thing had a difficulty setting that no even a Greek god could crack. What is with the XBOX version being toned down? My second playthrough was on the 360 and it was EASY PEASY. PC takes the medal for difficulty on this game.
There is a free online game called Dark Orbit that is pretty difficult to get good at. That's only hard because you are being hunted down like a rat... Constantly.
The game that takes the real crown for difficulty though is Ocarina of Time: Master Quest. The game that makes no sense. Cows in walls? Say what? I suppose it's supposed to be hard, though. The title kinda gives it away. I still beat it. I guess that makes me the master?

Anyhoo, I was trying to remember horribly difficult games I've played and tried to complete... Fall Out 2. That game hates you, I think. Or at least the developers do. Right out of the first village, you run into things that will kill you in one hit. And after only a short amount of time, the only reliable way of healing yourself gets taken away, I suppose because you're "taking too long." It's also incredibly hard to come across any items that are worth having, or money. I hear that the original Fall out is rather similar. The Elder Scrolls games can be a little like this this too, but no where near as bad.
I've played Battletoads and Contra as well (though it's been forever.) Mega Man games can be fairly difficult too, though they're beatable (although the only one I've legitimately beaten was Mega Man X, and Sigma was a bitch.) I tried playing Mega Man 9 for a little while.... Anyhoo...

I'm gonna have to agree with fantasy ender valkyrie profile: 2 was just unfairly difficult when compared to your character growth, it took forever to level people up and you're usually running around with at least ten characters. I remember having to grind like crazy for each boss i fought, ugh, I even looked at walkthroughs to see if there was actually a way to beat them without the grind, and while there were ways to do it they were so obsurdly complicated that my reactions would not be able to keep up (I'm talking to you, split the party and position them in different areas so you can constantly change the boss's target causing it to never unleash an attack, team splitting is not intuitive!)
