    "Holy" and "Paladin" are pretty much redundant. In many cases, so does divine.

    Title-wise, Paladin is pretty much the highest of the high. You can be a basic knight at the very lowest (word of wisdom: did ya know being a knight was, in the late Middle Ages, almost a religious-military ordinance?). You can be a crusader, even if there is no crusade to fight for (perhaps the health reform if you're an American, poverty in Africa, democracy in South America...actually, there are proper crusades around). Then, there's Paladin; the best of the best.

    I've yet to find something that tops Paladin in title importance. Maybe "Imperator", but that's not divine and more political, and you have to trascend your mortal self to become a warrior of the Host (and even then, if they allow you).
