The following is taken from wikipedia.org. "Three days after the shadows incident, only Otonashi, Yuri, Kanade, Hinata and Naoi remain, the others having already passed on. Per Kanade's idea, they hold a graduation ceremony, where they thank each other for their support. After the ceremony ends, Naoi takes his leave. Yuri makes her peace with Kanade before she leaves, followed shortly by Hinata. With Otonashi and Kanade left, Otonashi suggests that they could stay together to help others who come into the afterlife world, revealing in the process that he is in love with her. Kanade is hesitant to respond, as her one regret was never saying thanks to Otonashi, whose heart kept her alive in the real world due to the donor card he left behind when he died. Kanade thanks him and disappears in his arms, as Otonashi cries over the disappearance of his newfound love. Otonashi passes on shortly after. In an epilogue, in the world of the living, a girl resembling Kanade is humming "My Song" when a boy resembling Otonashi happens to pass by and recognizes the melody. When the girl turns to walk away, the boy chases after her to tap on her shoulder. In the BD/DVD-exclusive short "Another Epilogue", Otonashi remains in the school and becomes the new student council president, helping out other students to pass on." Now, it was said some time during the series that Otonashi happened to wander into the world because he had lost his memories. If Kanade had died (probably due to Otonashi's heart rejecting it's new body), then it sort of makes sense that Otonashi got there after Kanade. Maybe due to memory loss, he wandered for a very long time before finding the school. Personally, I think I would have liked "Another Epilogue" better.
But that would still mean that she was given the heart. And Otonashi didn't sign his donor card until just before his death. Still doesn't make sense. The dream would make more sense, but why could you see Yuri's memories in Angel's dream? As well as most other people?
I noticed that as well. But Angel said that she wanted to meet the person who gave her the heart. Maybe she was in a coma, and some people believe that while in a coma, you are actually in limbo (or whatever you call between life and death). The world they were in is a form of limbo. Or maybe, the entire thing was a dream that Angel was having during her surgery. When she finally left the world, she may have woken up from the dream. And if that's the case, I don't think I can justly explain why the others were in the dream. Maybe they were just a figment of Angel's imagination. They could be images of people she has seen in her life before the surgery took place. However, reason #2 doesn't make sense with after Angel left the world, an Otonashi reincarnation started following her because Angel was singing the song.
You know? I noticed a maaaaaaaaaaaaaaaaaaaassive flaw. If Angel received her heart from Otonashi, how is she there before him? She's been there since the beginning, and he is the last to show up. Something doesn't make sense if he was the one that gave her his heart.
